Health Improvement Team

The Health Improvement Team (HIT) works together with healthcare providers, residents, community based organizations and city leaders to assess health status, determine priorities, build action plans and provide leadership around health issues that impact the community.

We are a consistent presence in these communities and bring community knowledge to improve both patient care and community health. This work is centered on addressing the root causes of health inequities. Through this structural lens, our team brings partners together to identify social and environmental factors that influence health and works to build equitable programs, policies and systems that support every person to reach their best health.

Somerville Organizations -- You're Invited to Submit A Proposal

The Somerville Health Foundation is accepting proposals to:

  • Promote and support the provision of primary care and preventive health services accessible to
    the residents of the City of Somerville, Massachusetts
  • Promote and support educational, research and outreach programs that will improve the
    delivery and accessibility of primary care and preventive health services to the residents of the
    City of Somerville, Massachusetts.

Current funding priorities include: root causes of disease, health equity and/or health disparities, systemic racism and serving vulnerable populations across all ages among Somerville residents.

Proposals are due on August 16, 2022.
